CSWs focus on achieving goals in areas such as independent living, education, employment, social engagement, and recreation. These services are offered both in our office and within the community.

Clients occasionally encounter challenges that extend beyond clinical issues, often requiring assistance in developing essential life skills or accessing community resources. Community Support Services (CCSS) bridge these gaps by providing support. A Community Support Worker (CSW) helps individuals and families connect with the necessary services and resources to foster resilience. 

What are Comprehensive Community Support Services (CCSS)?

CCSS is a client-centered program designed to promote independence and strengthen connections with community resources, helping individuals succeed in education, employment, housing, and more.

Who can receive CCSS?

CCSS is available for individuals aged 5 and older who require additional support within the community and have Medicaid insurance coverage. Clients with a therapist outside of A New Hope are welcome to receive CCSS while continuing their therapy with their chosen provider.

What does CCSS entail?

Meetings with your Community Support Worker (CSW) will take place both at our office and in the community. The frequency and duration of these meetings will depend on your specific goals and needs. For example, a meeting may involve CSW support during a 504 or IEP meeting at your child’s school, assistance with completing an application at the housing department, or help with resume creation and job searching at the library. Your CSW will maintain communication with you and your therapist to monitor progress and ensure a coordinated support team.

What will this process look like?

After we receive your completed referral, our CSW will schedule a meeting with you to complete the intake process and identify your goals. These initial meetings will be held at our office.
